During my first playthrough I went to free Greirat from his cell after acquiring the cell key and when i went back to the cell all was found was his ashes. RIP Greirat. Patches was killed, Onion Bro was stuck in jail. With no one to save him, he lived the thug life, aimed high, died in sewers. To save Greirat from Irithyll, you have to save Siegward from the well outside the Church before the Cathedral of the Deep by buying his armor from Patches and giving it to him.. Fylm s

So I went ahead and started The Ringed City, fought the Demonic Prince with some help and when I came back to the Firelink Shrine...Greirat was there! If Siegward is still there, you're good to send Greirat. However, a boss needs to die in your world in a certain amount of time after sending Greirat or he dies. A really safe way of doing it when you know you need to is saving Curse-Rotted Greatwood or Old Demon King until sending him out, then going back ... Kill old Demon King (easiest boss to progress quest) RETURN TO FIRELINK - be sure to return to firelink to ensure Greirat has returned, failing to do so and progressing to irythill dungeon will result in his death. Greirat dies on his outing to Lothric. If you want to send Patches, send Greirat, reload firelink, then send Patches, reload firelink to make sure he's gone, then go on. If there is no rescue plan in place, by the time you reach the sewers (maybe earlier), Greirat is dead. Irithyll is easily the touchiest area regarding DkS3 quests, and if you're not careful, Greirat will die..
